BackgroundCancer patients often suffer from psychological distress during or after cancer treatment, but the use of psycho-oncological care among cancer patients is limited. One of the reasons might be that the way psycho-oncological care is organized, does not fit patients’ preferences. This study aimed to obtain detailed insight into cancer patients’ preferences regarding the organization of psycho-oncological care.Methods18 semi-structured interviews were conducted among cancer patients. Patients completed psycho-oncological treatment between 2015 and 2020 at the psychology department in a general hospital (...) or a center specialized in psychological cancer care in the Netherlands. The interview comprised questions related to preferences regarding the institute where to receive treatment, the psychologist who provides treatment, and the type of treatment, as well as questions related to experienced barriers and facilitators to receive psycho-oncological care. Interviews were digitally recorded and transcribed verbatim. Data were analyzed individually by two coders into key issues and themes.ResultsRegarding the institute, easy accessibility and prompt availability of psychol-oncological care were considered important. Regarding the psychologist, most participants had a strong preference to be treated by a psychologist specialized in cancer or other somatic diseases. Individual face-to-face therapy was preferred above other types of treatment. Several barriers were mentioned to receive psycho-oncological treatment, among which poor accessibility to psycho-oncological care, lack of knowledge on the possibilities for psycho-oncological treatment, and stigma. Most frequently mentioned facilitators were being assertive to ask for help, having a good relationship with the healthcare professional, and the integration of psycho-oncological support within medical cancer care.ConclusionFrom the patient’s perspective, the organization of psycho-oncological care for cancer patients should focus on easy accessibility and availability, delivered by specialized psychologists, and integration in medical cancer care. Online and group therapy are acceptable, but individual face-to-face therapy is preferred. It is warranted to increase awareness on psycho-oncological care targeting both patients and healthcare providers. (shrink)

Media use can empower people, provided that this is accompanied by a deeper understanding of the actors, processes and structures in the media sector – including media policy. It is, however, to be expected that media users’ literacy of media policy is rather limited. This is problematic as the absence of such understanding makes it impossible for citizens to hold the politicians they elected accountable for the media policy they develop. This article explores what media users know about media policy, (...) what they expect to know, and whether they care. We adopted a case-study approach, researching this question for the region of Flanders based on a combination of quantitative (representative survey) and qualitative (49 in-depth interviews) data. Recent defenses of shaming as an effective tool for identifying bad practice and provoking social change appear compatible with feminism. I complicate this picture by examining two instances of online feminist shaming that resulted in shame backlashes. Shaming requires the assertion of social and epistemic authority on behalf of a larger community, and is dependent upon an audience that will be receptive to the shaming testimony. Smajdor addresses the problem of inferior clinical outcomes among adults with impairments of capacity to give informed consent (AWIC). She notes that AWIC are generally excluded from clinical trials to protect them against harms and avoid exploitation and claims there is a causal link between involvement in clinical trials and favourable outcomes. She argues, given this link, that we should increase AWIC representation in clinical trials and can justifiably do so by recognising the capacity of AWIC to assent. AWIC form (...) a diverse group, with multiple aetiologies, including, for example, Down syndrome, traumatic brain injury and dementia. In some cases, the inability to consent may fluctuate so that the same person could be AWIC only at certain times. Yet despite these differences, the group has a shared unmet need. We agree with Smajdor that AWIC outcomes should be improved, but question whether simply increasing representation in clinical trials is sufficient to achieve this. We identify the following points: 1. AWIC are under-represented in clinical trials. The view that narrative artworks can offer insights into our lives, in particular, into the nature of the emotions, has gained increasing popularity in recent years. However, talk of narrative often involves reference to a perspective or point of view, which indicates a more fundamental mechanism at work. In this article, I argue that our understanding of the emotions is incomplete without adequate attention to the perspectival structures in which they are embedded. Drawing on Bennett Helm’s theory of emotion, I (...) argue that the narrative view fails to take into account the influence of perspective on the emotions. In order to address this gap in our understanding of the emotions, I highlight a mode of engagement with literature that prioritizes the perspectival features of a work. Focusing on lyric poetry, I argue that non-narrative artworks are those best placed to highlight this fundamental aspect of our emotional experiences. (shrink)

This paper is about a category of social conventions that, I will argue, have significant moral implications. The category consists in our conventions about what we notice and choose not to notice about persons, features of persons, and their circumstances. We normally do not think much about what we notice about others, and what they notice about us, but I will argue that we should. Noticing people is a way of engaging with them in social contexts. We can engage in (...) social noticing more or less respectfully, more or less benevolently. Moreover, our standard conventions about noticing often have disparate effects on different groups of people. To be noticed appropriately is to have one's moral and social standing affirmed; conversely, to be denied notice or to be noticed inappropriately is very often to be denigrated or objectified. My aim in this paper is to unpack these moral dimensions of conventions of noticing and discuss their implications for how people engage with each other in certain kinds of social exchanges involving persons with disabilities. In Part I, I explain what I mean by the conventions of noticing and how they operate in social interactions. I show that although we do not always attend to them, these conventions are thoroughly embedded in our everyday social life. In Part II, I argue that these conventions have important moral dimensions and as such, should be governed by moral principles and values. I employ a Kantian framework of duties of love and respect in order to show how moral concerns should shape the way we use conventions of noticing and respond to their use by others. In Part III, I draw out the implications of this picture for social interactions among strangers when one or more of the parties involved has an immediately visible disability. (shrink)

Most psychological literature on gaslighting focuses on it as a dyadic phenomenon occurring primarily in marriage and family relationships. In my analysis, I will extend recent fruitful philosophical engagement with gaslighting by arguing that gaslighting, particularly gaslighting that occurs in more public spaces like the workplace, relies upon external reinforcement for its success. I will ground this study in an analysis of the film Gaslight, for which the phenomenon is named, and in the course of the analysis will focus on (...) a paradox of this kind of gaslighting: it wreaks significant epistemic and moral damages largely through small, often invisible actions that have power through their accumulation and reinforcement. Many philosophers view it as an attractive feature of Lewis’s semantics for counterfactuals that it can easily be modified to avoid this principle. However, Walters and Williams beg to differ. They argue that Conjunction Conditionalization is an indispensable ingredient of any Lewisian semantics, since CC is entailed by standard Lewisian theorems and a plausible semantic claim about irrelevant semifactuals. If this is true, (...) the entire tradition of revisionist counterfactual semantics is misguided, and so are many philosophical theories in which counterfactuals play a role. We argue, in defense of the revisionist tradition, that Walters and Williams’ ‘plausible semantic claim’ is in fact anything but plausible. The concept of phronesis is venerable and is experiencing a resurgence in contemporary discourses on professional life. Aristotle’s notion of phronesis involves reasoning and action based on ethical ideals oriented towards the human good. For Aristotle, humans possess the desire to do what is best for human flourishing, and to do so according to the application of virtues. Within health care, the pervasiveness of economic agendas, technological approaches and managerialism create conditions in which human relationships and moral reasoning are becoming (...) increasingly de‐valued. This creates a tension for nurses, and nursing leaders, as the desire to do what is morally right is often in conflict with contextual demands. In this paper, Aristotle’s writing on phronesis is examined with a focus on his classic conceptions of eudaimonia, the virtues, deliberation, judgement, and praxis. Building on Aristotle’s work, a number of contemporary views are explored with a focus on what various conceptualizations offer for the discipline of nursing. These expanded conceptions of phronesis include attention to: embodiment in practice; open‐mindedness including the capacity to stay curious and open to recognizing what we do not know; perceptiveness as a disposition towards insight and aesthetic understanding; and reflexivity as an ongoing process of interrogation and inquiry into ourselves and our actions. Drawing on these concepts, we discuss the affordances of phronesis as a morally informed guiding force to attend to modern‐day challenges in nursing practice and nursing leadership. (shrink)

This article will introduce readers to current debates in the philosophy of poetry. This includes discussion of the need for a philosophy of poetry as distinct from a philosophy of literature, the (in)compatibility of poetry and philosophy, poetic meaning and interpretation, and poetry in relation to affect, emotion and expressiveness, which opens up discussion of wider forms of poetry from spoken word to signlanguage poetry. The article ends with suggestions for future directions of research in the philosophy of poetry. I (...) argue that as the philosophy of poetry is gaining interest, the previous debates that presuppose an understanding of poetry as taking form on the page and as having fixed aboutness ought to be abandoned in favour of an understanding of a poem in the affective space, with issues of reception, interpretation, distribution, and performance in play. When the cost-effectiveness of newly approved cancer treatments is insufficient or unclear, they may not (immediately) be eligible for reimbursement through basic health insurance in publicly funded healthcare systems. Patients may seek access to non-reimbursed treatment through other channels, including individual funding requests made to hospitals, health insurers, or pharmaceutical companies. Alternatively, they may try to pay out of pocket for non-reimbursed treatments. While currently little is known of these practices, they run counter to a deeply held egalitarian ethos that (...) is prevalent in many publicly funded healthcare systems. In this article, we investigate to what extent this ethos can be grounded in theories of justice, notably egalitarianism and prioritarianism. We argue that allowing out-of-pocket payments by patients themselves, in principle, is not unjust from the perspective of either of these theories, provided that it does not raise in-practice justice-based concerns, for instance by displacing more cost-effective care, to the detriment of other patients, or by failing to treat patients equally. In contrast, we conclude that the practice of making exceptions for individual patients by health insurers or healthcare providers does run counter to the justice-based requirements of equal treatment. This paper defends the liberal arts as an effective way to acquire habits of thought (creativity, skepticism), as opposed to skills. The ability to think creatively, historically, and skeptically can only be acquired slowly, socially, and with a diverse population. While this defense of the liberal arts (as opposed to a skills-focused defense) well supports some of the hallmarks of American liberal arts education (in person, bricks and mortar, not accelerated), it also has some critical implications for how the liberal (...) arts are taught. This dissertation is an attempt to loosen what I see as a chokehold by which two paramount assumptions constrict our epistemic endeavors. These Enlightenment assumptions--that we accept or refute ideas as true based on transparently clear and orderly methods and criteria, and that individuals accept or refute truth claims--are still central in epistemology, despite their many critics . Thinking about gossip as an epistemologically productive concept provides us with the means to critique those assumptions, and further attempts to broaden our (...) notion of an epistemological foundation. ;Gossip at first appears to be an unlikely candidate for such a resurrection, mainly because its treatment by academics has been dismissive; this dismissal is in part due to Enlightenment conceptions about truth and falsehood. Chapter One surveys the social science literature on gossip and rumor, revealing that social scientists begin with such restrictive definitions of what gossip is that their conclusions amount to little more than tautology. Chapter Two shows that humanists have a slightly different approach to gossip, but with roughly similar results. ;The handful of philosophers who deal directly with gossip or rumor almost as a unit accept uncritically a division between "purposive" conversation and "idle" chatter. To do so, I think, perpetuates a limiting epistemic foundation on a linguistic level. In contrast, I argue in Chapter Three that the very existence of something like gossip proves the inadequacy of the foundationalist myth , and that to attempt to understand and use gossip with foundationalist tools is simply a wrong fit. My understanding of gossip is based on this central fact: we undertake the activity of gossip or rumor-spreading because we are trying to make sense out of something--we need to collect knowledge socially. Gossip originates from dissonance; it acts as a counterweight to more official information, and can't be considered apart from official knowledge. We use gossip and rumor, along with more orthodox sources of information, to formulate our understanding of ourselves and the world around us. The extent to which gossip and rumor are spread is the extent to which the analysis is shared, and not individualized. Gossip is both a genealogical tool and an speculative tool. (shrink)

This innovative critical study reinterprets Ludwig Wittgenstein's philosophy for the study of modernist and contemporary literature and brings Wittgenstein into literary conversations around problems of difficulty, ethical instruction, and the yearning for transformation. Central to Karen Zumhagen-Yekple͹'s book are her critical readings of key modernist texts by Franz Kafka, Virginia Woolf, and James Joyce. Throughout, Zumhagen-Yekplé brings to bear an interpretive framework that she derives from Wittgenstein's gnomic "Tractatus Logico-Philosophicus" (first published in English in 1922, the "annus mirabilis" of (...) modernism), which she treats not as a theory of logic or metaphysics but as a complex mock-theoretical puzzle. The book's final chapter turns to recent fiction by J. M. Coetzee, a living author conscious of his debts both to Wittgenstein and his modernist literary precursors. This book will interest students of literary modernism, Wittgenstein, and the interconnections between fiction and ordinary language philosophy. (shrink)

Although prior work on ethical decision-making has examined the direct impact of magnitude of consequences as well as the direct impact of emotions on ethical judgments, the current research examines the interaction of these two constructs. Building on previous research finding disgust to have a varying impact on ethical judgments depending on the specific behavior being evaluated, we investigate how disgust, as well as happiness and sadness, moderates the effect of magnitude of consequences on an individual’s judgments of another person’s (...) unethical behavior. Specifically, we propose and find that because disgust and happiness are both associated with more heuristic-based processing, they both lead to a stronger reliance on the magnitude of consequences when forming ethical judgments. In contrast, because sad and neutral emotional states are associated with more systematic processing, they both result in a weaker reliance on the magnitude of consequences. As such, the effect of magnitude of consequences on judgments of unethical behaviors is stronger when individuals making the judgments are experiencing disgust or happiness versus sadness or a neutral state. This research shows that ethical judgment severity is contingent on individual-level factors, particularly the current emotional state being experienced by the individual, interacting with magnitude of consequences to impact the ethical decision-making process. This paper provides a Christian philosophical and theological response to the moral imperative of human advancement through multiplanetarism as emerging from the Great Filter theory. First, it explains the Fermi paradox that underlies the Great Filter theory. Next, it explores this theory itself and its motivational power in driving advancement toward multiplanetarism. In particular, we explore two motives behind the multiplanetary efforts that are (in part) guided by the Great Filter theory, analyzing the relevant statements of SpaceX founder and chief (...) engineer Elon Musk. Finally, we assess these motivations (and the accompanying normative assumptions) behind space exploration and multiplanetarism from the perspective of Reformational philosophy, using Herman Dooyeweerd’s theory of ground motives as a lens. Thus, the paper’s leading research question is this: How can we assess the current attempts at securing a multiplanetary existence for humankind in the light of Dooyeweerd’s theory of ground motives? Although databases have been well-defined and thoroughly discussed in the computer science literature, the actual users of databases often have varying definitions and expectations of this essential computational infrastructure. Systems administrators and computer science textbooks may expect databases to be instantiated in a small number of technologies, but there are numerous examples of databases in non-conventional or unexpected technologies, such as spreadsheets or other assemblages of files linked through code. Consequently, we ask: How do the materialities of non-conventional databases differ (...) from or align with the materialities of conventional relational systems? What properties of the database do the creators of these artifacts invoke in their rhetoric describing these systems—or in the data models underlying these digital objects? To answer these questions, we conducted a close analysis of four non-conventional scientific databases. By examining the materialities of information representation in each case, we show how scholarly communication regimes shape database materialities—and how information organization paradigms shape scholarly communication. These cases show abandonment of certain constraints of relational database construction alongside maintenance of some key relational data organization strategies. We discuss the implications that these relational data paradigms have for data use, preservation, and sharing, and discuss the need to support a plurality of data practices and paradigms. (shrink)

This article explores the processes through which Australian recipients select unknown donors for use in assisted reproductive technologies and speculates on how those processes may affect the future life of the donor-conceived person. I will suggest that trust is an integral part of the exchange between donors, recipients, and gamete agencies in donor conception and heavily informs concepts of relatedness, race, ethnicity, kinship, class, and visibility. The decision to be transparent about a child’s genetic parentage affects recipient parents’ choices of (...) donor, about who is allowed to “know” children’s genetic backgrounds, and how important it is to be able to “pass” as an unassisted conception. In this way, recipients must trust the process, institutions, and individuals involved in their treatment, as well as place trust in the future they imagine for their child. The current market for donor gametes reproduces normative conceptions of the nuclear family, kinship, and relatedness by facilitating “matching” donors to recipients by phenotype and cultural affinities. Recipient parents who choose not to prioritize “matching,” and actively disclose the process of children’s conceptions, may embark on a project of queering heteronormative family structures and place great trust in both their own children and changing social attitudes to reduce stigma and generate acceptance for non-traditional families. Public healthcare systems are increasingly refusing (temporarily) to reimburse newly approved medical treatments of insufficient or uncertain cost-effectiveness. As both patient demand for these treatments and their list prices increase, a market might arise for voluntary additional health insurance (VHI) that covers effective but (very) expensive medical treatments. In this paper, we evaluate such potential future practices of VHI in public healthcare systems from a justice perspective. We find that direct (telic) egalitarian objections to unequal access to expensive treatments based (...) on different ability to afford VHI do not stand up to scrutiny. However, such unequal access might lead to loss of self-respect among individuals, or loss of fraternity within society, rendering it more difficult for citizens to interact on equal moral footing. This would be problematic from a relational egalitarian perspective. Moreover, the introduction of VHI might turn out to have negative consequences for the comprehensiveness and/or the quality of the public healthcare services that are offered to all patients equally through basic health insurance. These consequences must be weighed against potential health gains and the value of liberty. We conclude that governments should be careful when considering the introduction of VHI in public healthcare systems. (shrink)

Bauer, Taub, and Parsi's review of an international sample of standards on informed consent, confidentiality, commercialization, and quality of research in tissue banking reveals that no clear national or international consensus exists for these issues. The authors' response to the lack of uniformity in the meaning, scope, and ethical significance of the policies they examined is to call for the creation of uniform ethical guidelines. This raises questions about whether harmonization should consist of voluntary international standards or international regulations that (...) include an official oversight mechanism and sanctions for noncompliance, and about who should participate in the harmonization process. Moreover, the normative assumptions and political dynamics that shape global policymaking need to be addressed. This commentary explores the policy implications and normative questions raised by the idea of international ethical guidelines for the use of biotechnologies and biotechnological resources such as stored samples of human tissue. Philosopher Judith Butler has influenced how people talk about vulnerable bodies and sees vulnerability as universal, existential, and relational. Being vulnerable is part of the human condition. The main theoretical areas that run across Butler’s work; power, knowledge and subjectivity, performativity, and ethics—are of particular relevance to nursing practice. This review aims to explore how Butler’s theoretical work is reflected in research literature within a nursing context. We conducted a scoping review guided by Arksey and O’Malley’s methodological framework. A systematic (...) literature search of CINAHL (EBSCOhost), MEDLINE (Ovid), Embase (Ovid), PsycINFO (Ovid), and Web of Science identified 15 papers. Butler’s theoretical work was applied at an individual and social level in research literature within a nursing context. Nurses need to reflect on their clinical practice and role as health professionals in relation to power and performativity in encounters with patients who are marginalized. Nurses’ working conditions, recognition, and understanding are strongly influenced by society, and calling nurses heroes undermines their capacity to challenge and resist the hero identity. The healthcare system’s impact on patient-nurse encounters challenges patients’ and nurses’ subjectivity, performativity, and power relations. The review allowed us to describe how Butler’s theoretical work can facilitate a reflection on nursing practice which is a prerequisite for caring, ethical relationships, and working conditions within a nursing context. Butler’s concepts can provide useful perspectives on how nurses understand, communicate with, and care for patients, as well as a nuanced understanding of the nursing role and power relations and structures. Tobacco mosaic virus has served as a model organism for pathbreaking work in plant pathology, virology, biochemistry and applied genetics for more than a century. We were intrigued by a photograph published in Phytopathology in 1934 showing that Tabasco pepper plants responded to TMV infection with localized necrotic lesions, followed by abscission of the inoculated leaves. This dramatic outcome of a biological response to infection observed by Francis O. Holmes, a virologist at the Rockefeller Institute for Medical Research, was used (...) to score plants for resistance to TMV infection. Our objective was to gain a better understanding of early to mid-twentieth century ideas of genetic resistance to viruses in crop plants. We investigated Holmes’ observation as a practical exercise in reworking an experiment, having been inspired by Pamela Smith’s innovative Making and Knowing Project. We had a great deal of difficulty replicating Holmes’ experiment, finding that biological materials and experimental customs change over time, in ways that ideas do not. Using complementary tools plus careful study and interpretation of the original text and figures, we were able to rework, yet only partially replicate, this experiment. Reading peer-reviewed manuscripts that cited Holmes’ 1934 report provided an additional level of insight into the interpretation and replication of this work in the decades that followed. From this, we touch on how experimental reworking can inform our strategies to address the reproducibility “crisis” in twenty-first century science. (shrink)

We examined the efficacy of including a research intermediary (RI) during the consent process in reducing participants' perceptions of coercion to enroll in a research study. Eighty-four drug court clients being recruited into an ongoing study were randomized to receive a standard informed consent process alone (standard condition) or with an RI (intermediary condition). Before obtaining consent, RIs met with clients individually to discuss remaining concerns. Findings provided preliminary evidence that RIs reduced client perceptions that their participation might influence how (...) clinical and judicial staff view them. This suggests that using RIs may improve participant autonomy in clinical studies. (shrink)
